We are looking for a relationship-driven Senior Home Safety Outreach Liaison to help us build referral partnerships in the senior care and healthcare community. This is not a clinical treatment role. Instead, this position focuses on community outreach, referral relationship development, education, and helping connect older adults and families with practical home safety solutions.

The Senior Home Safety Outreach Liaison will develop and strengthen relationships with referral partners such as occupational therapists, physical therapists, home health agencies, discharge planners, case managers, senior living communities, social workers, elder care professionals, and other community organizations serving older adults.

This role is ideal for someone who understands senior care, values aging in place, and enjoys building trust-based community relationships. Occupational Therapist experience is preferred, but candidates with strong senior care, rehab, home health, or community outreach backgrounds are also encouraged to apply.
